What is Choices?
Choices is Universal Provident’s private medical insurance product which gives you the power to choose the benefits that suit you and your needs. We have taken the benefits of traditional private medical insurance, added some extra benefits and broken them down into convenient modules, with each module being individually priced – so you are not only in control of the benefits you choose, but also the overall price of your cover.

How does Choices work
The starting point of building you Choices policy is to choose one of the core Modules –

Module A1 provides cover for in-patient and day-patient treatment allowing access to in excess of 450 hospitals nationwide, depending upon the hospital scale you wish to be covered under. The cover includes charges for hospitals fees (accommodation, nursing, theatre fees and so on), surgeons’ and anaesthetists’ fees, home nursing and many other benefits including a valuable cash benefit if you receive treatment free of charge under the NHS. In addition, unlike our competitors’ in- and day-patient covers, this module also extends to include the cost of diagnostic out-patient MRI, CT and PET scans.

Module A2 provides cover for private in-patient treatment if such treatment is not available free of charge under the NHS within 90 days of your specialist confirming that it is necessary. Treatment will be provided in a hospital approved by us within a 30 mile radius of your home and cover will include hospital charges, surgeons’ and anaesthetists fees and charges for radiotherapy, chemotherapy and oncology. If treatment is available free of charge under the NHS you will be entitled to a cash benefit for each night you spend in hospital.

Once you have chosen your core Module, the choice is yours –

You can purchase Core Modules A1 or A2 with no additional modules.

You can add any combination of additional modules to build the cover that meets your needs:

  • Module B* provides a full refund of eligible out-patient specialist costs
  • Module C* covers out-patient specialist charges of up to £500 per person per year
  • Module D provides cover for out-patient physiotherapy and certain complementary therapies including acupuncture, homeopathy and osteopathy, to a maximum of £500 per person per year.
  • Module E provides cover for in-patient, day-patient psychiatric treatment to a maximum of £250 per day for up to 28 days per year and out-patient psychiatric treatment for up to £450 per person per year.
  • Module F is the Employee Assistance Programme (EAP). This helpline can provide information, counselling and assistance on a wide range of issues as diverse as debt, stress, elderly care and consumer rights. This service is available to employees only.
  • Module G is Support+. In some cases involving major illnesses (such as cancer or stroke) it may help the member – or his/her family – to talk to someone with specialist knowledge of the condition and how it affects their life. This service is available to both members and their families (if insured).

*Note – if you require cover for out-patient treatment you should choose either Module B OR Module C, not both.

Once you have chosen the Modules to be included under your Choices policy, you need to make the following additional choices –

Who is to be covered – your cover can include yourself only, your spouse/partner (including same sex partners) and/or your children.

What hospital scale you require – if you have chosen cover under Core Module A1 you will need to tell us which of Scales A, B or C you wish to be covered under (Scale A covers the most expensive hospitals, mainly situated in London, Scale B covers the next tier of hospitals including many in London and other major cities and Scale C covers all other hospitals in the UK). The hospital scales are based purely on the fees charged by the hospitals and do not reflect the clinical services provided.

Do you want an excess on your policy – by choosing an excess on your policy, you can achieve substantial savings on your premiums. Excesses are available as follows and will attract the discount shown in the table:
