Acceptance Guidelines

For all products we will require a completed direct debit mandate. Please note however that we will only require a single direct debit mandate, irrespective of the number of products purchased, by which we will collect all premiums.

Premier Care, In-Care, Total Care

Minimum age on acceptance = 18 (other than dependent children)
Maximum age on acceptance = 74

Premiums based on the age of the oldest life to be covered

Medical Acceptance Terms – Full medical Underwriting or Moratorium
(Moratorium only for the Living Care and Dental Care sections of Total Care)

Documents Required – Application Form, Moratorium Declaration (if applicable),

Living Care, Dental Care and Travellers Care

Minimum age on acceptance = 18 (other than dependent children)
Maximum age on acceptance = 74 (70 for Traveller's Care)

Premiums based on the age of the oldest life to be covered

NHS PLUS

Minimum age on acceptance = 18 (other than dependent children)
Maximum age on acceptance = 74

Standard premiums apply irrespective of age (subject to maximum as above)

Income Care and Income Care Plus

Minimum age on acceptance = 18
Maximum age on acceptance = 54

Premiums are based on the occupation of the applicant
Premiums for Income Care Plus are additionally based on the age of the applicant

Corporate Care, Group In-Care and Group Total Care

Group size 1 to 4 employees
Minimum age on acceptance = 16 (other than dependent children)
Maximum age on acceptance = 74
Premiums are based on the age of oldest life to be covered within each family unit

Acceptance Terms – Full medical underwriting or moratorium (N.B. The Living Care and Dental Care sections of Group Total Care are only available on a moratorium basis.)

Documents Required – Company Application Form, Employee Application Form (Moratorium or Underwritten) and Moratorium Declarations (if applicable)

Group size 5 to 99 employees
Minimum age on acceptance = 16 (other than dependent children)
Maximum age on acceptance = 74 (age 75+ may be accepted on referral to Head Office)

Premiums are based on the age of the employee

Acceptance Terms – Full medical underwriting, moratorium or continued personal medical exclusions (CPME) – see below for details regarding acceptance of CPME business. (N.B. The Living Care and Dental Care sections of Group Total Care are only available on a moratorium basis.)

Documents Required – Company Application Form, Employee Application Form (Moratorium or Underwritten) and Moratorium Declarations, if applicable. (NB – see below for specific requirements relating to requirements for employees transferring on CPME terms.)

CPME Terms

  1. Continued Personal Medical Exclusions (CPME) terms are available to groups of 5 to 99 employees who currently hold a group private medical insurance policy.
  2. All groups accepted on this basis must be company paid (i.e. the employee’s premium is fully paid by the company, with no contribution from the employee).
  3. The policy must cover all employees within the chosen eligibility criteria (e.g. all employees with more than 12 month’s service, all senior managers etc.).
  4. Cover must be continuous between the existing contract and the Universal Provident contract.
  5. CPME terms are only available to those members transferring to Universal Provident at the start of the contract. Any subsequent joiners will be subject to either a moratorium or full medical underwriting.
  6. All members transferring on CPME terms must submit the existing insurer’s insurance certificate showing the full terms of acceptance and any specific medical exclusions applied. In addition the following documentation will be required for each employee:
    1. Groups of 5 to 9 employees – CPME application forms completed by each member, giving limited information on the member’s (and any dependants’) recent medical history. Additional terms may be required as a result of the answers given.
    2.  Groups of 10 to 99 employees – a Group Transfer Form giving brief details of all persons (including dependants) to be covered.
  7. For groups whose existing insurance is on a Medical History Disregarded (MHD) basis, the following additional rules apply:
    1. Transfer of MHD arrangements are only available to groups of 50 or more employees.
    2.  Claims experience for the current year and previous three years must be provided.
    3.  Acceptance and premiums are subject to underwriter’s approval
    4.  MHD is only available to those members currently insured on this basis who transfer at the start of the Universal Provident contract. Any subsequent joiners will be subject to either a moratorium or full medical underwriting.
  8. CPME terms are not available for Group NHS PLUS schemes.

Group NHS PLUS

Minimum age on acceptance = 18 (other than dependent children)
Maximum age on acceptance = 74

Acceptance Terms – Moratorium

Documents Required – Company Application Form and Employee Application Form (Group NHS PLUS)

All Group NHS PLUS business with over 99 members must be referred to Head Office for acceptance.
